Box breathing (4-4-4-4)
Four equal phases — inhale, hold, exhale, hold — used by everyone from clinicians to special forces. Simple to remember, easy to settle into.
Typical cadence
- Classic 4–4–4–4 (inhale–hold–exhale–hold)
- Advanced: 5–5–5–5 or tuned near ~0.1 Hz (~6 breaths/min)
- Nasal breathing, relaxed abdomen, low shoulders
How it works
Box breathing uses four equal phases (inhale–hold–exhale–hold). Slowing and evening the breath rate around ~5–6 breaths/min entrains cardiorespiratory rhythms near the baroreflex ‘resonance’ (~0.1 Hz), typically increasing heart rate variability (HRV) and Respiratory Sinus Arrhythmia (RSA), and supporting parasympathetic tone.
Evidence highlights
- Slow breathing (<10 bpm) increases HRV/RSA and relates to improved relaxation, comfort, and reduced arousal (systematic review).
- Breathing at ~5.5–6 bpm with equal in/out increases HRV in healthy adults.
- Inhalation/exhalation ratio and slower rates modulate HRV and perceived relaxation.
- HRV biofeedback at resonance (~0.1 Hz) strengthens baroreflex gain and can aid stress regulation.
How to practice
- Sit or lie comfortably; breathe through the nose if possible
- Inhale 4s • Hold 4s • Exhale 4s • Hold 4s • Repeat (2–5 min)
- Gradually lengthen counts toward ~5–6 breaths/min if comfortable
Safety notes
- If dizzy or uncomfortable, return to normal breathing
- Avoid excessive breath-holds if you have cardiovascular or respiratory conditions—consult a clinician if unsure
